Office Policies and New Patient Forms
Welcome to Integrative Health and Wellness! You will be directed to our new patient PDF forms after confirming that you have read and understand the information below. We want you to be comfortable starting your journey with us. To understand how the care we provide is different, please take your time to review our important notices and website carefully!
We ask that you complete each page of the comprehensive forms provided. Please bring any recent laboratory testing and/or functional testing (stool tests, hormone testing, food testing, nutritional evaluations etc.) to your first appointment.
If you are not conveniently able to locate these items and your provide requires them to continue care, we will have you complete a release for medical records at your first appointment.
We also ask that you bring the following to you initial consultation:
- Photo ID
- List of prescription medications.
- All supplements so that the ingredients may be reviewed.
You will be reminded via email of your appointment time and date. Please arrive 5-10 minutes prior to your appointment. If you arrive 10 minutes or more PAST your scheduled appointment time, you may be asked to reschedule to maximize your time with your provider.
We make every effort to give patients convenient appointments and do hold a cancellation list. To help us, we ask that you please notify the office 48 hours in advance if you need to change or reschedule your appointment. Please leave a voicemail if this time frame falls on the weekend, a holiday or after hours. Failure to show, or notification of less than 48 hours in advance will result in a no show/late cancellation fee. Late cancellations are 50% of your appointment cost and no shows are billed at the full cost.
Please know that we are unable to provide supervision for young children that accompany you to our clinic and we ask that you make other arrangements.

Our office hours are listed on the home page. They are subject to change and can vary if we are in appointments or out for meetings. It is always best to call ahead.
All of the supplements that are recommended at Integrative Health and Wellness are available for purchase in our office. You are not obligated to purchase supplements from our office. If you are going to stop by the office to pick up supplements we ask that you kindly call us ahead of time to make sure they are in stock and we are available to assist you. We do offer a large percentage of our products for sale on our website at www.wellnessmichiana.com/shop/. Local pick up or shipping can be selected.

We greatly appreciate our patients and work hard to ensure each patient is our top priority during appointments. We make every effort to respond to questions and/or concerns presented outside of an appointment within 2-4 business days.
PRESCRIPTION REFILL REQUESTS
For prescription refills, we ask that you call us and contact your pharmacy to have them fax or electronically send over the medication refill request. Our fax number is (574) 598-4445. It may take up to 48-72 business hours to process a prescription refill.
Please plan ahead to avoid any interruptions in your medications.
COPIES OF MEDICAL RECORDS & LABS FROM OUR OFFICE
You will be given a copy of your labs at each visit to keep for your records. Should you need an additional copy of your medical records, a $15 fee will be charged for copies and postage.
Our office accepts cash, check and credit cards for services rendered. When you schedule an initial visit, we request a credit card on file to hold the appointment for you. No charges will be applied to your credit card unless you miss or cancel an appointment without proper notice. On the day of your scheduled appointment, all charges for consultations, standard visits and other services will be reviewed in detail. Payment is due on the day of service.
You may also use pre-tax dollars from a Health Savings Account (HSA) to pay for your consultations and follow up visits.
We are passionate about providing you with the best possible care, unrestricted by the constraints of traditional insurance companies. Insurance companies are often not interested in preventative-wellness care, but rather are invested in traditional models of medicine. We are committed to treating the whole person and to better serve our clients we do not directly file with your insurance companies. We will provide you with an itemized bill containing traditional ICD-10 codes at the end of your visit. This super bill allow you to easily file with your insurance company. Visit the Patient Resource tab to view the insurance form and instructions on how to fill them out. Neither provider submits their medical notes to insurance companies. In addition, Erin and Blair are not Medicare providers.
We do not fill out medical disability forms for patients or submit medical notes to support them. On rare occasions we will write a letter to detail the medical necessity of testing or dietary changes for work or school. Under such circumstances, we will bill an hourly rate to write such letters.
